Instructions:  Select your desired search criteria, then click on the Find Providers button.  You may enter the first few characters of a provider name or ZIP code if you do not know the full name of a provider or would like to search for a range of ZIP codes.  Press and hold the Ctrl (or Apple) key while clicking in the multi-select boxes to choose more than one of the options.
